Studio Workshop Agreement

Activity: Wax Carving & Stone Setting Workshop

Location: 1627 N 2nd St, Studio 3 S, Philadelphia, PA 19122

1. Acknowledgment of Risk

I understand that participation in this wax carving and jewelry workshop involves the use of specialized tools, including but not limited to carving instruments, torches for soldering, and polishing equipment. I acknowledge that these activities carry inherent risks, such as cuts, burns, or other injuries. I voluntarily assume all risks associated with my participation.

2. Studio Safety

I agree to follow all safety instructions provided by the instructor, Paz Sandoval. I understand that failure to follow safety protocols or using equipment in a negligent manner may result in my removal from the workshop without a refund.

3. Release of Liability

I, the undersigned, hereby release, waive, and discharge Paz Sandoval and the Studio from any and all liability, claims, or demands for personal injury, property damage, or loss arising out of or related to my participation in this workshop, whether caused by negligence or otherwise.

4. Medical Consent

In the event of an emergency, I authorize the instructor to seek medical treatment on my behalf if I am unable to do so. I agree to be responsible for any costs associated with such medical treatment.

5. Personal Property

I understand that I am responsible for my own tools and materials (including stones) brought into the studio. The studio is not responsible for any lost, stolen, or damaged personal property.
